Forms:

     NOTES:

  1. .doc versions of forms are fill-in-able using Word, or Open Office, all you need to do is type the information into the highlighted fields, then be sure to save the file. You can then mail it to the person named on the AC&C 2009 Registration Form (If you're using Open Office, be sure Field Names - under the View menu - is turned off!)
  2. .pdf versions will print out blank and you can fill them in by hand
  3. the AC&C Chorus Registration Spreadsheet will open in Excel or in Open Office (spreadsheet); it allows you to type in the information, and if you put in the amount for an AEP for each person, the spreadsheet will calculate the total for you. (If the font seems very small, you may need to adjust the document zoom size to 100% or greater)
